- You must use a computer or device with a good internet connection, webcam, and microphone. If you run into issues connecting, restart your computer or check out the Doxy.me or help.doxy.me
- You must have an operating Android or iPhone with a good internet connection
- Complete your online PATIENT FORMS by clicking HERE . Please select ‘Existing Patient’ when prompted.
- Email your Picture ID and Insurance Card to (per office email) Fay@o2smiles.com
- A team member will be contacting you to collect payment in full (if applicable). If we are assisting you in filing your dental insurance, if they reimburse for services, you may request a refund for any credit.
